TAX INFORMATION
As
a self-employed freelance worker, I require the acquisition of tax form
1099 from clients who paid more than $600 for a single job. Form 1099
must be mailed to me before the tax filing deadline. In kind, I will
happily and immediately complete and return tax form W-9 at the request
